Application for Enrolment FORM

The CWTS Application for Enrolment is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO administrators and student services staff responsible for processing new enrolments, it provides a structured framework for collecting all student information required to establish a compliant enrolment record, meet AVETMISS reporting obligations, identify support needs, and initiate the student's training journey with accurate and complete data from the outset.
 
Your enrolment form is the single most important data collection instrument your RTO uses. Every downstream compliance obligation, from AVETMISS reporting and USI verification through to student support planning, certification, and audit evidence, depends on the accuracy and completeness of the information collected at enrolment. An enrolment form that misses required fields, collects information inconsistently, or fails to obtain the declarations and consents your RTO needs creates data quality problems that are time-consuming to correct and highly visible to ASQA when they surface in your NAT files or student records. Getting this right at the point of enrolment is significantly less costly than addressing errors after the fact.
 
This form provides a consistent, auditable process for student enrolment data collection across all programs and delivery modes your RTO offers. It aligns to clauses 2.1, 2.3, and 2.4 of the Standards for RTOs 2025 and was developed in conjunction with the Student Enrolment Policy Procedure and Student Information Policy Procedure. Prior education, employment, and language background fields for statistical reporting
Disability, LLN, and support needs identification prompts
Fee acknowledgement and payment arrangement confirmation fields
