Credential Review Checklist FORM

The CWTS Credential Review Checklist is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO administrators, compliance officers, and managers responsible for issuing and quality-checking certification documentation, it provides a structured framework for reviewing qualifications, statements of attainment, and records of results against the requirements of the Standards for RTOs 2025 and the Australian Qualifications Framework before any credential is issued to a student.
 
Credential issuance is one of the most visible and consequential compliance functions your RTO performs. A certificate that contains incorrect information, references a superseded training product version, omits a required AQF element, or is issued against an incomplete student record is an error that is difficult and time-consuming to correct after the fact. Where errors are identified by ASQA during audit rather than by your own quality processes, they reflect on the integrity of your entire certification practice. A structured pre-issuance review applied consistently to every credential your RTO issues is the most effective way to ensure that the documentation your students receive is accurate, compliant, and a genuine reflection of their achievement.
 
This checklist provides a consistent, auditable process for credential review across all qualifications and units of competency on your scope. It aligns to clauses 2.1 and 2.2 of the Standards for RTOs 2025 and was developed in conjunction with the Issuing Certification Policy Procedure and the Credentials and Certification Register. It includes the following:
 
Student identification and enrolment record completeness verification fields
Training product name, code, and version currency confirmation section
AQF certification documentation content requirements review prompts
Assessment outcome completeness and authorisation confirmation fields
Issuing authority sign-off and credential register update confirmation section
Reissue and amendment recording fields for audit trail purposes
