Assessment Outcome Report FORM
The CWTS Assessment Outcome Report is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for trainers, assessors, and RTO administrators responsible for recording and communicating student assessment results, it provides a structured framework for consolidating assessment outcomes across a unit of competency, documenting the basis for each judgement, and managing the reassessment process where a student has not yet demonstrated competency.
Recording assessment outcomes accurately and consistently is a compliance obligation that sits at the intersection of your assessment, certification, and AVETMISS reporting requirements. An outcome recorded incorrectly, incompletely, or without sufficient documentation of the evidence on which it was based creates a chain of downstream errors that affects your student records, your NAT file, and ultimately the credential your student receives. Where reassessment is required, the process must be documented, the grounds for the additional attempt must be clear, and the outcome of the reassessment must be recorded with the same rigour as the original assessment judgement.
This form provides a consistent, auditable process for recording and managing assessment outcomes across your scope of registration. It aligns to clauses 1.3 and 1.4 of the Standards for RTOs 2025 and was developed in conjunction with the Assessment Evidence Collection Policy Procedure and the Standards of Assessment Guide Template. It includes the following:
