Continuous Improvement Record Sheet FORM

The CWTS Continuous Improvement Record Sheet is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO staff, trainers, assessors, and administrators at all levels of your organisation, it provides a structured framework for capturing and recording individual staff contributions to your RTO's continuous improvement process, ensuring that improvement ideas, identified issues, and suggested changes are documented consistently and fed into your broader quality management system.
 
Continuous improvement is most effective when it is embedded in the daily practice of every person in your organisation, not just managed from the top down by compliance staff. Trainers who identify a gap in a resource, administrators who notice an inconsistency in a process, and assessors who recognise a recurring student difficulty all have valuable improvement insights that your RTO needs a mechanism to capture. Without a structured, accessible way for staff to record and submit those observations, improvement opportunities are lost to informal conversation and your continuous improvement register reflects only the issues that management is already aware of rather than the full picture of what your RTO needs to address.
 
This form provides a consistent, auditable process for collecting staff-generated improvement contributions across all areas of your RTO's operations. It aligns to clauses 4.1, 4.2, 4.3, and 4.4 of the Standards for RTOs 2025 and was developed in conjunction with the Continuous Improvement Policy Procedure and the Continuous Improvement Register.
