Course File Checklist FORM

The CWTS Course File Checklist is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO administrators, compliance officers, and trainers and assessors responsible for reviewing student records prior to credential issuance, it provides a structured framework for confirming that all required administration and assessment evidence is in place before a qualification or statement of attainment is issued.
 
Issuing a credential against an incomplete or non-compliant course file is one of the most consequential errors an RTO can make. Missing enrolment documentation, incomplete assessment records, unresolved USI issues, and gaps in evidence collection all create certification errors that are difficult to correct after the fact and highly visible to ASQA during audit. A systematic pre-credential review process using a consistent checklist is the most effective way to catch these gaps before they become findings.
 
This checklist provides a consistent, auditable process for course file review across every student completing a qualification or unit of competency on your scope. It aligns to the Standards for RTOs 2025 and was developed in conjunction with the Monitoring Training and Assessment Practice Policy Procedure and the Assessment and Evidence Collection Policy Procedure. It includes the following:
 
A structured review of enrolment documentation completeness
Assessment evidence sufficiency and validity prompts aligned to the rules of evidence
USI verification and AVETMISS data accuracy checks
Credential issuance authorisation fields for audit trail purposes
A findings and action record for documenting and resolving identified gaps prior to certification
 
This form is suitable for use as a pre-certification quality gate, as part of your internal audit process, or in preparation for an ASQA audit.
