Assessment Appeals Review FORM

The CWTS Assessment Appeals Review Form is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO compliance officers, quality managers, and independent reviewers responsible for managing and documenting the formal assessment appeals process, it provides a structured framework for conducting, recording, and resolving a student's appeal against an assessment process or outcome in a way that is fair, consistent, transparent, and free from conflicts of interest.
 
An assessment appeal is one of the most significant quality assurance events your RTO can experience. When a student disputes an assessment decision, they are asserting that the process used to reach that decision was flawed, that the evidence was not fairly evaluated, or that the outcome does not accurately reflect their demonstrated competency. Your RTO's response to that assertion must be structured, impartial, and documented from the point the appeal is received through to the final outcome and any corrective actions that follow. The Standards for RTOs 2025 require that your appeals process is accessible, conducted by a person with no prior involvement in the original assessment decision, completed within a reasonable timeframe, and capable of producing a documented outcome that the student can rely on and that ASQA can review.
 
This form provides a consistent, auditable process for managing assessment appeals across all programs and units of competency on your scope. It aligns to clauses 2.7 and 2.8 of the Standards for RTOs 2025 and was developed in conjunction with the Complaints and Appeals Register and the Assessment Appeals Policy Procedure.
