Certification Re-issue Request FORM

The CWTS Certification Re-issue Request Form is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO administrators and student services staff responsible for managing requests from past students for replacement or duplicate certification documentation, it provides a structured framework for receiving, verifying, processing, and recording re-issue requests in a consistent and auditable way.
 
Former students request re-issue of their certification documentation for a wide range of legitimate reasons, including lost or damaged certificates, name changes following personal circumstances, employer verification requirements, and further study applications. Your RTO's obligation does not end at the point of initial issuance. You must be able to locate the original student record, verify the accuracy of the credential to be re-issued, confirm the identity of the person making the request, and produce a replacement document that meets the current AQF certification requirements, regardless of how long ago the original was issued. An undocumented or inconsistently managed re-issue process creates both a consumer protection risk and an audit exposure, particularly where re-issued credentials contain errors that differ from the original record.
 
This form provides a consistent, auditable process for certification re-issue management across all past students and program completions on your record. It aligns to clauses 2.1 and 2.2 of the Standards for RTOs 2025 and was developed in conjunction with the Credential Review Checklist, Credentials and Certification Register, and Issuing Certification Policy Procedure.
