Hazard / Incident / Injury Report FORM

The CWTS Hazard, Incident and Injury Report is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO staff, trainers, and administrators responsible for identifying and recording workplace health and safety events, it provides a structured and consistent process for documenting hazards, incidents, and injuries as they occur across all training environments your RTO uses.
 
Accurate and timely hazard and incident reporting is a legal obligation under the Work Health and Safety Act 2011 as well as a compliance requirement under the Standards for RTOs 2025. RTOs that rely on informal reporting, verbal communication, or inconsistent documentation of WHS events cannot demonstrate the structured safety management system that clause 1.8 requires. More practically, incidents that are not properly recorded cannot be analysed for patterns, cannot inform your continuous improvement process, and cannot support a defensible position if a WorkSafe or regulatory investigation follows a serious event.
 
This form provides a consistent, auditable process for hazard and incident reporting across your entire operation. It aligns to clause 1.8 of the Standards for RTOs 2025 and was developed in conjunction with the Work Health and Safety Policy Procedure and the WHS Management Plan. It includes the following:
