Professional Development Request FORM

The CWTS Professional Development Request Form is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO staff, trainers, and assessors seeking approval to undertake professional development activities, and for the managers responsible for assessing and approving those requests, it provides a structured framework for documenting the details of a proposed professional development activity, establishing its relevance to the staff member's role, and recording the outcome of the approval process.
 
Professional development in an RTO context is not simply a staff benefit. It is a compliance obligation. Trainers and assessors must maintain their vocational currency and professional practice on an ongoing basis, and your RTO must be able to demonstrate that it is actively supporting and recording that development. A structured request and approval process ensures that professional development activities are relevant to the staff member's role and your RTO's compliance needs, that the cost and time investment is assessed and authorised before it is committed, and that approved activities are captured in your CPD records in a way that is traceable and auditable.
 
This form provides a consistent, auditable process for professional development planning and approval across all staff roles in your organisation. It aligns to clauses 3.2 and 3.3 of the Standards for RTOs 2025 and was developed in conjunction with the Professional Development Policy Procedure, CPD Record Book Template, and Trainer and Assessor Register.
