Supervision Arrangements Declaration FORM

The CWTS Supervision Arrangements Declaration is a fully editable, rebrand-ready form aligned to the Standards for RTOs 2025. Designed for RTO managers and compliance officers responsible for managing trainer and assessor compliance, it provides a structured framework for establishing, documenting, and declaring the supervision arrangements that apply to trainers and assessors who do not yet fully meet the requirements of the Standards for RTOs 2025 in their own right.

 

Supervision arrangements are not an informal courtesy extended to new or developing staff. They are a formal compliance mechanism that your RTO must have documented and actively implemented wherever a trainer or assessor is delivering training or conducting assessment without independently meeting the full requirements of clauses 3.1, 3.2, and 3.3 of the Standards for RTOs 2025. The nature of the supervision, the identity of the supervising practitioner, the scope of activities being supervised, and the timeframe and conditions for the arrangement must all be clearly documented. An arrangement that exists in practice but not on paper provides no audit evidence and no protection if a question arises about the validity of assessment conducted under that arrangement.

 

This form provides a consistent, auditable process for establishing and recording supervision arrangements across your training workforce. It aligns to clauses 3.1, 3.2, and 3.3 of the Standards for RTOs 2025 and was developed in conjunction with the Trainer and Assessor Competency Policy Procedure, Trainer and Assessor Register, Trainer and Assessor Competency Matrix Template, and Trainer and Assessor Risk Assessment Form. It includes the following:

 

  • upervised trainer and assessor identification and current competency status fields
  • Supervising practitioner details and qualification confirmation section
  • Scope of supervised activities and delivery contexts recording fields
  • Supervision method, frequency, and monitoring arrangement details
  • Timeframe, review milestones, and conditions for transition out of supervision
  • Declaration and sign-off fields for both parties for audit trail purposes
